Report of Accident or Near Miss incident on Club premises or during an event organised by the Club

Please raise a ticket to report an accident that takes place on any Club Property or during an event organised by the Club. Also to report a near miss incident. A report should be raised regardless of whether or not there is any injury. Submitting these reports will assist the committee to identify any issues and risks.
If the person involved is not a club member a Management Committee member or other club member should be requested to log on to the portal and submit a report on behalf of the person concerned.
